Patient’s can contact us at (978) 922-2226 once order and records are received to schedule a new patient consultation. 

For New Patient Inquiries

If you are new to our practice, we require patients to be referred by another physician for a neurology consult.  Please have your Primary Care or other referring physician fax over the Neurology Consult Order and any past medical records to our secure fax at (978) 922-2269.

If you’re looking to re-establish Neurological Care and are new to our practice, please have all neurological records sent to our secure fax at (978) 922-2269.

We do not accept Workers Compensation Insurance or Motor Vehicle Insurance. 

If you are being referred due to a Motor Vehicle Accident, we will need a copy of the Exhausted PIP Letter from your motor vehicle insurance carrier prior to scheduling an appointment
