Getting Ready For Your First Visit

Complete Your Digital New Patient Intake

Seven days before your appointment, you’ll receive a secure electronic questionnaire by text or email. Please complete this intake form before your visit to help us prepare for your care.

To save time at check-in, we encourage you to:

  • Complete questionaire in advance
  • Upload a photo of your insurance card
  • Pay your copay in advance

Electronic intake forms must be completed in advance of check-in at every visit. If unable, you may complete them on an in-office iPad with staff assistance.

What to Bring

Please make sure to bring the following items with you:

  • Our Electronic questionnaire sent via SMS or email must be completed prior to the appointment or patients must arrive 20 minutes early to complete on the iPad in the office.
  • Your insurance card
  • Copies of any imaging (on a CD if available)
  • Records from any neurologists you’ve seen before, including prior neurological testing (these can also be sent to us in advance)

Arrive Early & Review Our Cancellation Policy

Please arrive 15 minutes before your appointment to complete the check-in process. Arriving early might not be needed if Healthnote is completed.

Due to our extended wait times and to avoid no shows, we require 24 hour notice on all cancellations or reschedules. After two missed appointments (less than 24 hour notice), patients are considered for discharge.


Appointment Confirmation & Cancellation Policy

  • Please confirm your appointment by 1:00 PM on the business day prior. Unconfirmed appointments may be released to other patients.
  • Provide at least 24 business hours’ notice for cancellations (or 72 hours for procedures like EEG, EMG or Infusion Therapy).
  • Failure to provide proper notice on two occasions may result in discharge from our care.

f your insurance provider isn’t listed, we recommend calling your plan’s member services to confirm your out-of-network benefits before scheduling your appointment.

We do not accept the following coverage:

  • Masshealth Standard Plans
  • Motor Vehicle Accident Insurance
  • Third Party Liability Insurances
  • Worker’s Compensation

Payment Options

    • Personal checks
    • All major credit cards
  • Self-Pay Patients
    • Patients without insurance coverage are welcome to contact our office to discuss self-pay options prior to their appointment.
    • Payments will be due at the time of service.

If your insurance or coverage has changed:

Please notify our office before your appointment so we can verify your benefits ahead of time. This helps ensure your visit is covered and prevents delays at check-in. If we are unable to verify coverage, your appointment may need to be rescheduled.
