Getting Ready For Your Visit

In order to make the most of your visit with us we ask that you arrive 15 minutes or more prior to your appointment to complete our new patient intake form. This can also be completed ahead of your appointment by clicking on the attachment below.

We require 24 hour notice on all cancellations or reschedules. After two missed appointments (less than 24 hour notice), patients are considered for discharge.

You’ll need the following on your first visit to the office:

  • Any relevant information regarding your health history
  • Copies of any imaging on compact discs
  • Insurance card
  • List of your current medications
  • New Patient Intake Form and New Patient Consent Form
  • Photo ID
  • Records from any previous neurologists seen and any prior neurologic testing.

If your insurance is not listed above, please contact your insurance member service department to verify your benefit coverage prior to scheduling an appointment in our office.

We accept cash, personal checks and all major credit cards. 

Please note we do NOT accept the following insurances in our office:

  • Masshealth Standard Plans
  • Motor Vehicle Accident Insurance
  • Third Party Liability Insurances
  • Worker’s Compensation
