What is GUIDE?

The Guiding an Improved Dementia Experience (GUIDE) model is a new Centers for Medicare and Medicaid (CMS) program that aims
to reduce strain on unpaid caregivers, and enable people living with dementia to remain in their homes and communities.

The program is funded by Medicare for enrolled patients. North Shore Neurology is working with Craniometrix to deliver this
caregiver support.

This model provides individuals and families with an interdisciplinary team to provide care navigation, education, support and
respite care.

GUIDE Benefits for Patients

*Respite care is available for individuals enrolled in the program that have moderate to high dementia care needs and receive
support from an unpaid caregiver, such as family members

What support is offered through the GUIDE program?

The Guide program is built around 9 service areas which we have partnered with Craniometrix to deliver:

  • Comprehensive assessment
  • Care planning
  • Ongoing monitoring and support
  • Medication reconciliation and management
  • 24/7 helpline access
  • Care coordination and transitional care management
  • Referrals and coordination for services and support
  • Caregiver education and support
  • Respite services
  • North Shore Neurology will still provide all medical care for the patient. This includes any medication, tests or other care related
  • to the patient’s dementia.

What does Craniometrix do?

Craniometrix will provide support for caregivers. They will also help the caregiver access services to meet the patient’s social needs.
This support may include:

  • Up to 70 hours of annual respite care funded by Medicare (subject to assessment) to support caregivers in taking a break.
  • 24/7 access to advice and support
  • Regular assessment of patient social needs and caregiver needs
  • Education and support groups for caregivers
  • Support accessing community services

SiteRx – Increasing Access to Clinical Research in all Settings

What does this mean for you? 

Clinical trials are a fundamentally important component of the clinical care landscape. Through our partnership, we can offer clinical research as an additional care option.

Acting as an extension of our practice, the SiteRx clinical trial team of experts will:

  • Guide you through the journey of exploring clinical trials.
  • Provide detailed information about specific studies that may be a fit for you.
  • Answer any questions you may have about the process.

Is participation required? No. 

Participation in any clinical research opportunity is completely voluntary.

What to expect: If our clinical team identifies a research opportunity that may benefit you, we may recommend you for a research trial. In these cases, a member of the SiteRx team may reach out to you directly to discuss the details. If you receive a call from them, please know that it is a legitimate referral from our office.
